The Montgomery County Department of Health and Human Services Office of Public Health is conducting its first Community Health Assessment (CHA). The assessment will help the County better understand our community’s health status, concerns, and priorities. Your responses will help shape our Community Health Improvement Plan and the direction of public health in Montgomery County for years to come.

If you prefer a paper copy, limited hard copies of the survey will be available for pick up at Borough Hall or the Community Center at the Fellowship House.
All surveys are anonymous and confidential, but the information gathered will help make a difference in the health and well-being of Montgomery County. The survey takes about 15 minutes to complete and includes questions to help us understand your health priorities, your perspective on health issues your community is facing, and experiences you have had that may influence your health and well-being.
This work is critical in helping to meet the needs and priorities of all our residents. Please complete the survey by April 25.
